Retatrutide (LY3437943) is the most-watched unapproved compound in metabolic research. This overview covers its mechanism, the published evidence as it stands, and the open questions that make it an active research subject rather than settled science.
Mechanism: engineered triple agonism
Retatrutide is a 39-amino-acid synthetic peptide, built on a GIP backbone with a C20 fatty-diacid for albumin binding, that activates three receptors at once: GIP, GLP-1 and glucagon. The activity ratio is deliberate — strongest at GIP, moderate at glucagon, attenuated at GLP-1 relative to the native hormones. The design hypothesis: GLP-1/GIP suppress intake and improve insulin dynamics, while glucagon adds an energy-expenditure and hepatic fat-oxidation component the other incretins lack.
The published evidence
- Phase 1 established once-weekly pharmacokinetics and tolerability consistent with the incretin class.
- Phase 2 (NEJM, 2023) — the headline data: mean weight reduction ~24% at 48 weeks at the highest dose, without plateau at study end. Adverse events were predominantly gastrointestinal, consistent with class effects.
- Liver-fat sub-study: over 80% of participants with fatty liver at baseline reached normal liver-fat content at the top doses — a result driving substantial hepatology interest.
- Phase 3 (TRIUMPH program) is ongoing across obesity, diabetes, and obstructive sleep apnea populations; no phase 3 results have published yet.
Open research questions
Three dominate the current literature: whether glucagon-driven energy expenditure holds up against compensatory intake in large populations; heart-rate elevation observed at higher doses; and lean-mass preservation during rapid weight loss. Each is an active in-vitro and preclinical research area — receptor-level signaling studies of the triple agonism are still being mapped.
